This questionnaire is prepared for the benefit of both the prospective owner and the dog. It will better enable Mid-Iowa Boxer Rescue to place the right dog into the right home. We are looking for the best home to suit the specific needs of each dog. We are looking for permanent, lifetime homes.

Are they on heartworm preventive 12 months of the year?
Do you understand a Mid-Iowa Boxer Rescue volunteer must visit your home prior to approval to adopt?
Do you understand that the adoption fee is not refundable after a two-week grace period?
Adoption fees range from $350 to $450

I understand that if the information contained herein is found to be in any way false, my application will be refused or the dog adopted shall be relinquished to Mid-Iowa Boxer Rescue without a refund of monies paid.
